Photo Session Waiver

Pricing effective January 1, 2025

Additional Fees & Information:

You will receive a QR code from the makeup artist to secure your date. Please ensure you complete the process promptly to confirm your booking. If you would like additional guests to attend the session, please let Ms. Hicks know ahead of time.

Please note that any additional retouching, such as body thinning, eye enhancements, heavy blemish removal, or teeth placement, will incur an additional fee of $50. Basic retouching includes skin softening and light thinning.

Retouching is $95.00 per image. Any additional individuals in the image will be $55.00 per person.

Please arrive on time for your session. We appreciate a call if you are running late to hold your scheduled time. After the 15-minute grace period, your appointment will be automatically canceled and rescheduled for a later date. Deposits are non-refundable or transferable.

The images will be provided on a private website for your viewing. You will have 24 hours to make your selection. After this time, the images will be removed from the private site and will not be available again. The turnaround time for retouched images is two weeks from the date of your selection.

Upon selecting images, the four-digit numbers on the requested images must be sent to 240-280-4064.

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.

Jackie Hicks Portrait Artist “Jackie Hicks” dba Fond Memories has put preventative measures in place to reduce the potential spread of COVID-19; however, Fond Memories cannot guarantee that the virus will not be transmitted to you and/or your child(ren) causing you to become infected with COVID-19. Further, Fond Memories has taken all the necessary steps to clean and disinfect Fond Memories studio. It will be sanitized after each client by using disinfectant and cleaning agents recommended by health care professionals.

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I, my spouse child(ren) and/or any agents that accompany me to Fond Memories may be exposed to or infected by COVID-19 as a result of utilizing our Portrait and Makeup Service at Fond Memories and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that there is a risk of becoming exposed to or infected by COVID-19 and that I should come prepared to have my temperature checked.

I will be provided with appropriate protective equipment including but not limited to shoe covers, disposable cape, and if needed gloves. The Photographer and Make-up Artist will be in full protective gear: Masks, Face Shield, Disposable Cape, shoe covers, and gloves. There will be no large parties or groups permitted in Fond Memories.

Once you have arrived at the studio please call Jackie Hicks on 240-280-4064 to notify her of your arrival. It is imperative that you do this so that she can instruct you on how to enter the studio for your photo session.

Please check here that you understand only members that are in the photo session are allowed in studio. Please refrain from bringing extra guest as they will not be allowed in studio and will have to wait in their vehicle.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my child(ren) and/or myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I or my child(ren) may experience or incur in connection with my utilizing the services of Fond Memories.

On my behalf, and on behalf of my children, I hereby release, agree not to sue, discharge, and hold harmless Fond Memories, and its employees, agents, and representatives, of and from the claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of Fond Memories, its principal, employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after utilizing services of the Fond Memories.
